Please see page 2 for the instructions as per the Criteria. Once completed on the front page, please take this Preference form together with the Baptism Certificate to the Priest for them to complete the section “Completed by the authorised agent”. Once completed, please send this to the school together with a copy of the Baptism Certificate, if you have not already supplied a copy of this to us.

5.4

Significant familial Audit (i.e. Aunt, Uncle etc.)

(a) Please complete this form on page 1 and then in accordance with the Criteria instructions on page 2. The significant familial adult needs to complete the section at the bottom of page two and you as the parent/guardian need to also complete the section relating to you at the bottom.

(b) Please also complete the “Supporting Evidence Form – Section A”. This form is to be completed by the significant familial adult and you as parent/guardian and then the significant familial adult will need to take their Baptism Certificate and these two forms to their parish office for the Parish Priest to complete Section B.

Then please forward the Preference Form and a copy of their Baptism Certificate to the school.

For one or both of a child’s non Catholic parents that are preparing to become a Catholic: Please complete the front page and take this form to the Priest of the Parish you are attending to become a Catholic.
